Submission Information:

If you are interested in trying out for any of our varsity athletic programs, you must first complete the necessary pre-participation forms found below. Print, complete and return forms to:

This is required before any tryout, practice, or competition process begins. If you have any questions regarding preliminary requirements, please contact the athletic department at 717-948-6744.

Required Forms:

All students trying out and/or participating on a varsity athletic team must complete and sign the forms below:

  1. Participation Agreement Form (PDF)
  2. Medical Insurance Form (PDF)
  3. Sport Medicine Information Form (PDF)

In addition to completing the following two documents you must have your family physician or the penn state harrisburg team physician performing your physical exam complete and sign the last page of the pre-participation exam form and complete and sign the athletics medical clearance form.
