HIST Minor Application Form

Instructions: Use this form for any of the minors administered through the Weaver Building office. Be sure that you have filled in the blanks completely and accurately before submitting it. Incomplete or inaccurate applications cannot be processed. Important: If you have any questions about the minor, its requirements, or how these requirements may apply to your case, be sure to discuss them with the person in charge of the minor before submitting this form.

Applicant Information

Courses Needed for Completion of the Minor

List each course or category of courses needed for completion of the minor and the total credits for that item (e.g., "HIST 174," "400-level courses," etc.). Also, for each course or category, note any substitutions approved by the person in charge of the minor. It is not necessary to fill in each box in this section.

 

 

 

 

 

 

Approval Information

Note: All minor applications are subject to the approval of the student's academic advisor and the person in charge of the minor. The information in this form will be sent to these people electronically.
